Hazards

As a DHA contractor and as a Person Conducting a Business or Undertaking (PCBU), you have a responsibility to mange risks, by identifying hazards and implementing appropriate controls for any matters that you have direct control of.

How do I report a hazard?

From time to time, you may encounter a hazard for which you have no ability to control. For DHA managed properties, DHA is the Person with Management or Control (PWMC), and as such we need to be advised of any hazards on our sites that you have no management or control of.

When you see a hazard:
Step 1: Contact us on 139 DHA (139 342) to report the hazard.
Step 2:  Contact your local Property Manager to report the hazard so that they can assist with managing the hazard or contact the tenant. 
Step 3: Complete the Contractor Hazard Report form and return to the WHS team via email as soon as possible.

Incidents

What incidents do I need to report?

All DHA contractors providing maintenance or business-related services must report all incidents and near-misses to DHA, no matter how minor they may seem. We need to be able to determine if an incident is notifiable to a regulator.

If DHA has to report an incident, you will also need to notify your relevant regulator.

How do I report an incident?

When you see an incident or a near-miss:

Step 1: Call DHA on 139 DHA (139 342) to report the incident or near-miss.
Provide the following information:
  • what occurred
  • details about any injured persons and how they were medically treated
  • the root cause and contributing factors to the incident
  • control measures to be implemented to prevent a reoccurrence, and 
  • a progress update on the control measure implementation.
Step 2: Complete the Contractor Incident Report form. Return the form to the WHS team via email as soon as possible.
