| Business from Home Request (PDF 532 KB) | Complete this form when seeking approval to operate a business from home. This form applies to members in a Service Residence, Rent Band Choice or Members Choice property. |
| Ceiling increase (PDF 498 KB) | To apply for an increase to the ceiling where a property cannot be found within your ceiling. This form applies to Rent Allowance tenancies. |
| Flexible housing trial (PDF 507 kB) | Use where an extended housing benefit location has been approved and where approval is being sought to partake in the Flexible Housing Trial. This form applies to members in a Service Residence. |
| MCA SR clearance (PDF 483 KB) | Completed if you are moving out of Member Choice Accommodation or a Service residence and do not not require DHA's assistance to provide another housing solution. |
| Notification of housing change (PDF 567 KB) | DHA staff will advise you if this form needs to be completed when you are making a change to your housing at your own expense, or if you are relocating from overseas. |
| Owner/Member statement (PDF 504 KB) | This document can be used in lieu of a formal lease document. This form applies to Rent Allowance tenancies. |
| Property search (PDF 482 KB) | Used in locations where Defence has not set a Rent Ceiling to assist DHA in determining the rent ceiling. This form applies to Rent Allowance tenancies. |
| Release clause for tenancy agreements (PDF 101KB) | When securing a rental property it is advisable to include a release clause to assist with breaking your lease agreement for Service reasons or in the event of being posted. This form applies to members securing a rental allowance property. | SR rejection form (PDF 484 KB) | Complete this form if rejecting available Service residences. This form applies to members that are eligible to occupy a Service residence but wish to reject the Service residences available in online services. Note: If you are a member who has accompanied resident family or recognised other persons, you must reject any available Service residences if seeking rent allowance approval. |
| Tenancy After Discharge (PDF 482 KB) | This form is to be completed and returned when seeking approval for tenancy after discharge. This form applies to members in a Service residence. |
| Visitor notification (PDF 486 KB) | If you have a visitor come to stay for more than 5 weeks. This may affect your RA amount. This form applies to Rent Allowance tenancies. |
